Judson Center Health is currently accepting new patients and takes most insurance plans, including Medicare and Medicaid. Since opening in 2019, a majority of the clinic’s patients have been from Macomb County. However, the clinic welcomes residents from all over Metro Detroit. The facility is one of a select group of primary care practices in Michigan offering integrated healthcare that treats the body and mind in one location.
“I believe we have made a difference in the community,” said Haberkorn. “Everybody who walks in is a primary care patient. Primary care is focused on the whole person, and that includes their behavioral health including depression and anxiety and other types of conditions. It’s unique and wonderful that we’re able to provide all of these services and don’t have to send the patient somewhere else.”

Elizabeth Haberkorn, MSN, FNP-BC is a family nurse practitioner and the associate medical director for MedNetOne. She is an adjunct clinical instructor at the University of Michigan School of Nursing. Haberkorn received a Bachelor of Arts from Albion College in Exercise Physiology, a Bachelor of Science in Nursing from Madonna University, and a Master of Science in Nursing degree in Primary Care Family Nurse Practitioner with a concentration in Occupational Health nursing. Currently, she is a Doctor of Nursing Practice candidate with anticipated graduation in May, 2024.
Savannah Walukonis MSN, FNP-BC, CEN is a family nurse practitioner. She specializes in primary care and health promotion, medication-assisted treatment (MAT), and substance use disorder (SUD) services.
Sherry Silwanowicz, MA is a medical assistant and the practice manager for MedNetOne. She has 34 years of experience in a medical office setting. Silwanowicz’s responsibilities include direct patient care; implementing and monitoring office policies, procedures, and workflows; and coordinating multiple programs with providers and multilevel operations teams.
Michelle Thomason, RN, CARN has been a registered nurse for more than 10 years. She serves as the clinic’s medication-assisted treatment nurse care manager. Thomason received her nursing degree from St. Clair County Community College and recently attained certification in Addiction Nursing. Thomason’s nursing experience includes medical/surgical, home healthcare, complex care management, and medication-assisted treatment for substance use disorders.
Cecilia Garcia, CMA (AAMA), CHW is a certified medical assistant and community health worker. She is a member of the American Association of Medical Assistants and serves as a Spanish medical interpreter. Garcia schedules patient appointments, coordinates care between healthcare providers, provides resources and referrals, arranges transportation, and heads up the clinic’s patient outreach.
