Discount Programs
Sliding Scale Discount Programs
Judson Center provides services to all patients regardless of the person’s ability to pay. If assistance is needed, Judson Center offers a sliding fee schedule for the uninsured and under-insured who meet federal eligibility guidelines.
Judson Center sliding fee scale discounts use the Federal Poverty Guidelines.
If your family qualifies for discounts, you will be assigned the appropriate discount based on the below table.
Payment will be due on each visit and will be applied to all services provided.
Proof of Income
In order to apply for the program, proof of income is required to determine the appropriate discount for your family. Income is defined as wages before taxes from all sources. These include:
- Gross Wages, tips, and salaries
- Regular payments from public assistance
- Regular payments from social security
- Regular payments from unemployment
- Regular payments from worker’s compensation
- Regular payments from veteran’s benefits
- Regular payments from child support
- Regular payments from military family allotments
- Government employee pensions
- Private pensions
- Regular insurance or annuity payments
- Income from dividends, interest, rents, royalties
- Income from estates and trusts
Patients (Consumers) interested in applying for the sliding discount can complete the Sliding Discount Application and submit with proof of income at their appointment.

Insurance & Payment
Good Faith Estimate
You have a right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Judson Center is available to provide patients (consumers) who do not have insurance or who are not using insurance a reasonable estimate of what they will be billed for services.
- You are eligible to receive a Good Faith Estimate of the expected cost of services.
- You have the right to request Judson Center provide a Good Faith Estimate before scheduling an appointment.
- Make sure to save a copy or picture of your Good Faith Estimate.

Private Insurance
Judson Center accepts most major insurance plans. To find out if Judson Center is an “in-network” provider for your plan, please contact your insurance company.
Medicare
Judson Center accepts Medicare and Medicare Advantage Plans.
Michigan Medicaid
Judson Center accepts Michigan Medicaid and Medicaid HMO plans.
Discount Program
Judson Center also offers a special discount program for those patients that meet specific requirements. For more information, review the discount program.
