Judson Center’s integrated primary care clinic, Judson Center Health, earns Patient-Centered Medical Home (PCMH) designation from Blue Cross Blue Shield of Michigan

Warren, Mich. — October 23, 2020—Judson Center, a multi-county human service agency and Certified Community Behavioral Health Clinic (CCBHC), is pleased to announce its integrated primary care clinic, Judson Center Health, has been selected as a Patient-Centered Medical Home (PCMH) designated practice by Blue Cross Blue Shield of Michigan(BCBSM).

The PCMH model is founded on the central patient/physician relationship, with the primary care physician’s office serving as the patient’s “medical home” and the primary care physician’s team coordinating all of a patient’s medical care to improve patient health and outcomes.

A first-time PCMH designee, Judson Center Health opened in 2019 through a joint venture with MedNetOne Health Solutions and is located in Judson Center’s Warren facility. It is led by a primary care physician and nurse practitioner who have immediate access to the behavioral health services offered by Judson Center in the same Warren location.  Virtual visits are also available for both primary care and behavioral health.

The BCBSM announcement of PCMH designated practices was made on Sept. 15, 2020. BCBSM’s PCMH program is the largest of its kind in the country, and its PCMH-designated practices consistently outperform non-designated practices in clinical quality and utilization measures. Learn more here.

The news of the PCMH designation comes as Judson Center begins a partnership with The Salvation Army Eastern Michigan Division, providing behavioral health and drug addiction treatment and post-treatment services and onsite primary care to individuals at The Salvation Army Harbor Light Macomb facility in Clinton Township. Harbor Light offers an entire continuum of substance abuse treatment services, including in-patient medication assisted detoxification, 14- to 90-day residential treatment, outpatient treatment, case management, peer recovery services, legal aid, and pastoral care.

The goals of the Judson Center/Harbor Light partnership are reducing hospital and out-of-home placement readmissions for individuals who have been treated for mental health or substance use disorder, delivering outpatient mental health services to those who have not received treatment in the prior six months, reducing critical events such as hospitalizations, suicide, and overdoses, and tracking health effectiveness data and information set (HEDIS) measures on a quarterly basis for clients served.
